After discharge, patients may choose to
take advantage of a wide variety of follow-up
and aftercare services offered by the hospitals.

Intensive Outpatient Services are provided by a multi-disciplinary
team that individualizes assessments and treatments. Patients can be
treated in this program while still under the care of their private
physician.
Signs and symptoms that an evaluation is needed include:
Memory Loss
Altered Thoughts
Paranoia
Social Isolation

Disorientation
Combative Behavior
Suicidal Thoughts

Inability to Concentrate
Sleeping or Eating Disorders
Depression, Hopelessness, Pessimism
Restlessness and/or Hyperactivity

The Partial Hospitalization Program (PHP) helps prevent decompensation
and the need for rehospitalization. The program is designed for those
who require an intensive, structured, daily therapeutic environment,
but do not require 24-hour care. PHP is not only a cost effective
alternative to inpatient treatment, but also helps patients transition
from inpatient to outpatient care.
Individual treatment is designed to provide the opportunity to gain
insight, increased expression, problem solving and improved coping
skills.
