Patient Rights
You have the right to considerate and respectful care and treatment, which at all times maintains your personal dignity and is given without discrimination against your race, religion, sex, handicaps or national origin. You have the right to individual assessment and an individual treatment plan including periodic review. You have the right to actively participate in planning for treatment in the least restrictive environment as is possible.
You have the right to receive information necessary for you to give informed consent for any of the following:
- Voluntary admission to the hospital;
- Release of confidential information;
- Use of photography and/or audiovisual equipment;
- Use of routine and investigational medications and treatment procedures including explanation of benefits, risks and side effects;
- Transfer to another facility;
- After-care plans;
- The reason for admission;
- The proposed treatment;
- Purpose of treatment;
- Common side effects thereof;
- Alternative treatment modalities;
- Approximate length of stay (anywhere between 3-12 months, depending on the individual).
You have the right to personal privacy within the constraints of your individual treatment plan, including the right to communicate freely and privately with family and friends unless this is therapeutically contraindicated. You have the right to send and receive mail without hindrance, unless this is therapeutically contraindicated. If your right to communicate by any means is restricted, you and your family have the right to be informed of this by hospital staff, and any such restrictions will be documented and explained in your medical record. The restriction of your right to communicate must be ordered by a physician and will be reviewed by the physician and clinical staff at least every seven (7) days.
The right to the possession of clothing and personal effects shall be respected. The facility may take temporary custody of such effects when required for medical and safety reasons. A patient’s clothing and personal effects shall be inventoried upon their removal into temporary custody. Copies of this inventory shall be given to the patient and to the patient’s guardian advocate or representative and shall be recorded in the patient’s clinical record. All of the patient’s clothing and personal effects held by the facility shall be returned to the patient immediately upon the discharge or transfer of the patient from the facility, unless such return would be detrimental to the patient.
You have the right to expect the maintenance of confidentiality of communication between yourself and staff, and of all information recorded in your medical record.
You have the right to be transferred or discharged for medical reasons only, for your welfare, for the welfare of other residents, or for nonpayment of your stay (except as prohibited by Title 18 of the Social Security Act). You will be given reasonable notice of such a transfer or discharge to ensure its orderliness, and such notice and actions will be documented in your medical record.
You have the right to use the telephone at any time to report an alleged abuse. The ABUSE HOTLINE telephone number is 1-800-962-2873. Upon your request, hospital staff will assist you in dialing this number. The ABUSE HOTLINE telephone number and procedures for reporting alleged abuse are posted on each hospital unit.
You have the right to report concerns related to care, treatment, services and/or patient safety issues to The Joint Commission on Accreditation of Healthcare Organizations.
You have the right to be informed of Patient Rights in terms you understand. A copy of this document is given to each patient.
