First Home Care
First Home Care believes in enabling children, adolescents, families and adults
to receive the care they need in their homes and communities. Our programs reduce
the need for hospitalization and out-of-community placement, and offer freestanding,
independent levels of care. However, they also function as contracted, wraparound
services. Using strengths-based and family-focused approaches, we develop programs
that build on and improve the successes of our clients.
Therapeutic Foster Care Programming
Therapeutic Foster Care families provide for children and adolescents who require
out-of-home placement. We offer various differential levels of service tailored
to meet the individual needs of our youth. Through the services of specially
trained treatment families, the program provides comprehensive, coordinated,

Case Management Services
Case Management serves youth who are transitioning to or from intensive services.
Our case managers work with the youth, family, referring agency, and providers
to seek the least restrictive and most appropriate environment. We draw on each
youth's individual strengths as well as assessment data to determine appropriate
placement. Case management services include assessment and evaluation, treatment
plan monitoring and development, linkage to community-based services, monitoring
of services, advocacy, resource development, and outcomes development.
Home-Based Services
First Home Care's Home-Based Services offer intensive behavioral health intervention
for children, adolescents, adults and families. Designed to meet the specific
needs of the payor and its clients, Home-Based Services augment mental health
services delivered in community-based settings. These services include Crisis
Intervention and Assessment, Supportive Counseling, Medication Monitoring, Case
Management, Education, comprehensive discharge planning and other services designed
to address the individual needs of clients and families served.
Therapeutic Group Home
First Home Care operates group homes for youth who cannot function in the family
setting but who do not require institutional care or residential treatment.
Supervised by a clinically trained group home staff, the residents are expected
to attend public school or work in the community. The group home staff provides
individual and group counseling, training in independent skills, and emphasis
on responsibility to self and the group.
